Today, one of the most complained about skin problems is blemishes. Whether the cause is the sun, old age, pregnancy, hormone therapy or birth control pills, skin blemishes are a very common and disturbing problem. Contrary to popular belief, the problem of spots is not only seen in white-skinned people. Dark-skinned people are also at risk. The gradual damage to the ozone layer means that we are all more exposed to ultraviolet rays. This makes sunspots one of the most common aesthetic problems. The spot problem, which is encountered in all skin tones ranging from light to dark brown and is more common with advancing age, occurs in the areas of the skin most exposed to the sun. The most important step in spot treatment is to get to the root of the problem. Knowing what causes the skin to produce more melanin, the substance that gives the skin its brown color, i.e. hyperpigmentation, is extremely important in the success of the treatment.

WHAT IS SUNSPOT (MELASMA)?

Sunspots, called melasma in medical language, usually appear starting in the 20s and are more common in women. The spots are often bilateral, that is, on both sides of the face and symmetrical. The female hormone estrogen is one of the main causes of brown spots that appear on the forehead, cheeks, nose and upper lips after sunbathing – especially unprotected sunbathing when the sun’s rays are direct. Especially pregnancy, hormone therapy or birth control pills trigger the problem because of the rising estrogen hormone level. Although the color of the spots usually darkens in summer and becomes lighter and fainter in winter, they may remain the same darkness all year round.

One of the factors determining the success of spot treatment is whether the spots are deep or superficial. Superficial blemishes, which occur in the upper layers of the skin, are easier to treat, while blemishes in the lower layers of the skin are relatively more difficult. In general, deep and superficial spots are found together. By the way, it is worth noting that dark and wheat-skinned people are more prone to spotting than white-skinned people.

WHY DOES IT HAPPEN?

In fact, there are the same number of color cells in the sunspot area as everywhere else on the skin. The only difference is that color cells work harder in these areas and dark spots occur because color production is higher. There is some evidence that melasma may be related to stress. Hormones released during stress are thought to trigger color cells. However, as mentioned above, one of the main factors that trigger spot formation is hormonal factors. Pregnancy, taking birth control pills and hormone therapy predispose to spotting.

WHERE DO SPOTS OCCUR THE MOST?

It can be seen in all parts of the body with the advancement of age, especially in areas where the skin is sensitive and most in contact with the sun and chemicals. Hands, face, neck and décolleté are the most affected areas because they are the areas that come into contact with the sun and chemicals the most.

WHAT TO DO TO PREVENT SUNSPOTS?

People suffering from sun spots should regularly use sunscreen in summer and winter. It is also very important that the sunscreen has a factor of at least 30 and that they do not neglect to refresh it every 4 hours during the day. In addition, even if they apply sunscreen, they should not be exposed to direct sun too much, they should take care to stay in the shade, and they should use hats and glasses. They can use foundation or powder with sun protection factor. Thus, both the spots are camouflaged and the treatment is supported.

*Sunscreens with protection factor should be used in all seasons.
*Care should be taken to avoid uncontrolled burns on the skin while trying to tan in the sun or solarium.
*Do not use wet wipes, hair bleach, deodorant, perfume and tanning products before going out in the sun or while sunbathing. These products cause stains on the skin that are difficult to treat.
*Be careful to protect your skin from the sun after procedures such as waxing, laser epilation, skin care, peeling, especially on areas where the skin is thin, such as the face, armpits, groin area.
*Indiscriminate treatments applied to acne and acne scars invite blemishes.
*It is extremely important to protect from the sun during pregnancy. The interaction of hormonal changes with the sun prepares a perfect ground for the formation of spots.
* Regularly moisturizing the sensitive areas of the skin – face, décolleté and hands – both prevents blemishes and makes those areas more resistant to signs of aging.

HOW ARE SUN SPOTS TREATED?

*It is very important to determine the depth of the spot before treatment. The newer the formation of the spot, the better it responds to treatment. The older the stain, the longer the treatment period.
*The cause of the stain must be investigated and the factor that triggers the stain problem must be eliminated. Otherwise, it will be difficult to get a response from the treatment or the stain will reappear after a while, even if the stain lightens with the treatment.
*Spot treatment is not performed when there are acute problems such as burns, redness, itching or flaking on the skin. These problems should be treated first, and spot treatment should be started when the skin calms down well.
*It is extremely important to use sunscreen with at least SPF 30 factor during spot treatment.
* Different treatments such as laser systems, chemical peeling and mesotherapy can be used alone or together in spot treatment. The treatment protocol can be supported with spot creams and oral antioxidant supplements.

MESOTHERAPY

Mesotherapy is the injection of a formula of antioxidant and spot lightening substances such as vitamin C, glutathione, transexamic acid, salmon extract under the skin with fine needles. More successful results can be obtained by supporting the application with treatments such as laser or chemical peeling.

CHEMICAL PEELING

There are many different chemical peeling applications used in cosmetic treatments. The chemical peeling process, which gently peels the top layer of the skin, suppresses the pigment synthesis of pigment-producing cells and triggers cell renewal by increasing collagen synthesis. The session interval varies between 7 and 20 days depending on the product used; the number of sessions varies between 2 and 6 sessions depending on the treatment and the reaction of the skin. The patient can return to social life immediately after the procedure. Regular sessions not only lighten the appearance of blemishes, but also fine lines and wrinkles and acne scars.

LASER SPOT TREATMENTS

In laser spot treatment, lasers that either act on color pigments or peel and renew the skin are used. The mechanism of action of the first group is designed to damage the pigments in the lower layers of the skin by shooting at them. Since they do not damage the surrounding tissues, they do not cause any damage to the skin and they work without detaching the patient from his/her social life.
