In Brief
A Brazilian Butt Lift is a body contouring operation in which fat removed by liposuction from areas with excess fat is purified and injected into the buttocks. The goal is not only to add volume to the buttocks but to reshape the transition between the waist, hips and buttocks for a curvier, more proportionate silhouette. Because no implant is used, the result looks and feels natural.
For more information about fat transfer, the basis of this method, you can visit our Fat Injection page.
It is suitable for people with volume loss, flattening or sagging in the buttocks; enough fat in the waist, abdomen, back or hip area to transfer; good general health; and a stable weight. In very thin patients enough fat may not be obtained; in that case alternatives such as buttock implants are considered.
In smokers, fat survival decreases and wound healing becomes more difficult; therefore smoking must be stopped before and after surgery.
Before surgery, the areas from which fat will be taken and the areas of the buttocks to receive volume are marked while standing. First, fat is removed by liposuction from areas such as the waist, abdomen, back and hips. The harvested fat is purified of blood and fluid with special processing. It is then transferred to the buttocks through small entry points with fine cannulas, creating many thin tunnels. The incisions are a few millimeters long and fade over time.
The most important safety principle in BBL surgery is that fat is injected not into the gluteal muscle but only into the subcutaneous layer . The cannula is always kept above the muscle, ultrasound guidance is used when necessary, and the amount of fat transferred in a single session is limited with the patient’s safety in mind. This approach significantly reduces the most serious risks of the operation.
Swelling, bruising, pain and numbness are expected temporary effects. Part of the injected fat may be absorbed in the first months; therefore some patients may need a second session. Fat necrosis (firmness), infection, fluid collection, contour irregularities and asymmetry may occur. The risk of fat embolism, a rare but serious complication, is minimized by having the operation performed by an experienced surgeon, with a subcutaneous technique, in a hospital setting.
Assessment
Body shape, fat distribution and buttock form are assessed; the target silhouette is planned together.
Surgery
Fat removed by liposuction is purified and transferred to the buttocks in the subcutaneous layer in thin tunnels.
First 2–3 weeks
Sitting directly on the buttocks and lying on your back are avoided; a special cushion and compression garment are used.
Months 3 – 6
Swelling resolves, the surviving fat becomes permanent and the final shape emerges.
A Surgeon’s View
In BBL, a safe operation matters as much as a beautiful result. We always inject fat above the muscle, in the subcutaneous layer, and we do not aim for excessive volume in a single session. Natural, proportionate buttocks provide satisfaction far longer than exaggerated volume.

Contact UsThe fat cells that survive are permanent as living tissue. However, significant weight gain or loss also affects the shape of the buttocks; maintaining your weight is therefore important.
It is recommended not to sit directly on your buttocks for the first 2–3 weeks. When necessary, a special BBL cushion that shifts the load to the back of the thighs is used.
In patients with enough fat, BBL is usually preferred because it both slims and shapes and uses natural tissue. For very thin patients, the implant option is considered.
The amount varies according to body structure, skin elasticity and the desired result. For safety, an excessive amount is not injected in a single session; a second session is planned if needed.
Light walking begins in the first days. Heavy exercise and glute training are usually resumed after 6–8 weeks.
