In Brief

  • Excess skin on the upper lid and bags and hollows on the lower lid are addressed.
  • The upper lid scar stays in the natural crease, invisible when the eyes are open.
  • Lower lid bags are often repositioned into the hollow rather than removed.

Upper Lid: Relief from Heaviness

With age, the skin of the upper eyelid becomes excessive and hangs over the lashes. This makes the eyes look small and tired, makes applying makeup harder and, in advanced cases, narrows the field of vision. Removing the excess skin and, if needed, the fat pad in the inner corner redefines the eyelid crease.

Lower Lid: Balancing Bags and Hollows

Two problems are usually seen together on the lower lid: fat bags bulging forward and a hollow just below them. In the old approach the bags were removed; however, over time this could make the eyes look more sunken. Today, instead of removing the fat, it is usually preferred to spread it into the hollow to smooth the surface. If the bag alone is the problem, the incision is made on the inside of the eyelid and no scar is left outside.

Not Ignoring the Brows

In some patients, the real cause of eyelid heaviness is drooping brows. In this case, simply removing skin from the eyelid does not solve the problem and may even pull the brow lower. For this reason, brow position is always assessed as well and, if needed, a brow lift is planned together.

Risks

Temporary dry eye, watering, mild asymmetry and incomplete closure of the lids in the first days may occur. Because removing too much skin can rarely pull the lower lid down, restraint is essential in this operation.

Step by Step

1

Day of surgery

Upper lid surgery is usually done under local anesthesia; you go home the same day.

2

First 48 hours

Cold compresses and sleeping with the head elevated reduce swelling.

3

Days 3 – 5

Stitches are removed.

4

Week 2

Bruising largely fades; makeup and social life can resume.

A Surgeon’s View

If you have dry eye complaints before surgery, be sure to mention them. This information is important for planning how much skin to remove and for arranging aftercare.

Frequently Asked Questions

Lower lid results are very long-lasting. On the upper lid, new age-related skin laxity may develop again within 10–15 years.

Usually after 1–2 weeks, once the eyes are completely comfortable.

Because your eyes may tire in the first days, it is recommended to use them with short breaks.

Because it removes a tired, heavy look, the face is perceived as fresher overall.
