In Brief
There are four main causes of under-eye darkness, and only one of them responds well to filler:
Through a single entry point on the upper cheek, filler is placed in very small amounts in a deep plane close to the bone using a flexible, blunt-tip cannula. This technique reduces the risk of bruising and vessel injury. A total of 1 ml or less is usually enough for both eyes.
Under-eye skin is very thin. Filler injected in excess or too superficially can create a bluish tint or the appearance of persistent swelling. In people prone to fluid retention, the filler’s water-binding property can increase puffiness. This area always calls for restraint.
Assessment
The cause of the darkness is determined.
Application
Small amounts of filler are placed in a deep plane with a cannula.
First days
There may be mild swelling; sleeping with your head elevated and reducing salt helps.
Week 2
The result settles; a small touch-up is done if needed.
A Surgeon’s View
Volume loss in the cheekbone area often plays a role in a tired under-eye look. Supporting the midface allows less filler to be used under the eyes.

It results from the filler being injected too superficially; if needed, it is corrected by dissolving the filler.

Using a cannula reduces the risk; if bruising occurs, it fades within a few days.
