In Brief
The ears reach most of their adult size around the age of 5–6. This is also the time when children start school and their relationships with peers intensify. Surgery before school can therefore prevent possible teasing and exclusion. The child’s own wishes and discomfort must also be taken into account when deciding on surgery.
There is no upper age limit for prominent ear surgery. Many adults who have covered their ears with their hair for years and avoided photos are freed from this discomfort with a short procedure under local anesthesia.
A small incision is made in the natural crease behind the ear. The missing fold is recreated in the cartilage with permanent sutures; if needed, a deep ear bowl is reduced and the ear is brought closer to the head. The aim is not to pin the ears to the head but to create a natural angle.
In some patients with soft cartilage and mild deformity, incision-free thread techniques can be used. However, in prominent ears with stiff cartilage the risk of recurrence is higher. In terms of permanence, open surgery is the most reliable option.

A bandage that protects the ears is worn.
Weeks 1 – 2
The bandage is removed and you return to school or work.
Month 1
A headband that protects the ears continues to be worn at night.
Week 6
Contact sports can be started.
A Surgeon’s View
In newborns, the ear cartilage is very soft in the first weeks thanks to the mother’s hormones. Molding started during this period can eliminate the need for surgery later.

The operation is performed under general anesthesia; mild pain afterward is controlled with simple painkillers.
Because the scar stays in the crease behind the ear, it is not visible from the outside.
