In Brief

  • These lines deepen as the cheek tissues slide downward.
  • Overfilling the line directly can create a heavy look.
  • The goal is not to erase the line but to reduce its depth.

Why Does This Line Deepen?

The smile line is actually a natural contour of the face. With age, as the cheek tissues slide down, bone support decreases and skin elasticity declines, tissue accumulates over this line and it deepens.

A Smarter Approach

Simply filling the inside of the line may look good in the short term, but when done with a large amount it can create a heavy, unnatural look around the mouth. A better approach is to first support the cheekbone area to lift the sagging tissue, then correct the remaining part of the line with a small amount of filler.

Safety

The area around the nostril contains important blood vessels. Injection depth and technique are very important here; using a cannula reduces the risk.

Step by Step

1

Assessment

The depth of the line and midface volume are assessed together.

2

Support

If needed, filler is first placed in the cheekbone area.

3

Correction

A small amount of filler is applied to the remaining part of the line.

4

Follow-up

The result is evaluated after two weeks.

A Surgeon’s View

The aim should not be to make the smile line disappear completely; young people have this line too. A softened line preserves the face’s natural expression.
