In Brief

  • Breast enlargement can be caused by glandular tissue, fat or a combination of both.
  • Glandular tissue does not shrink with exercise or diet; it must be removed surgically.
  • The scars are very small and hidden at the edge of the nipple.

Why Isn’t the Gym Enough?

Many men come to us frustrated after months of chest workouts without results. The reason is that a significant part of the enlargement comes not from fat but from breast glandular tissue. As the muscle grows, the glandular tissue may be pushed further forward and the appearance may become more noticeable. Glandular tissue disappears only when it is surgically removed.

Causes

  • Hormonal changes in puberty and older age
  • Excess weight and weight fluctuations
  • Certain medications, anabolic hormones and supplements
  • Systemic conditions such as liver, kidney and thyroid diseases

If there is an underlying cause, it must be treated first. That is why hormone tests and breast ultrasound are performed when necessary.

Treatment Approach

In most patients, two procedures are performed together: the surrounding fat is thinned with liposuction through millimeter-sized incisions, and the firm glandular tissue is removed through a small incision at the edge of the nipple. Patients with significant skin sagging after major weight loss may also need excess skin removed.

Psychological Relief

Gynecomastia can significantly affect social life, especially in young people. After surgery, patients most often mention being able to wear T-shirts, feeling comfortable at the gym or on the beach, and improved posture.

Step by Step

1

Assessment

The ratio of glandular to fat tissue is determined by examination and, if needed, ultrasound.

2

Surgery

Liposuction and removal of glandular tissue are performed together.

3

First week

A compression vest is worn; most people return to work in 3–5 days.

4

Week 6

Weight training is resumed and the chest contour becomes clear.

A Surgeon’s View

Breast enlargement that begins in puberty often regresses on its own within 1–2 years. If it persists beyond this period, an assessment is recommended rather than waiting.
