In Brief

  • The degree of sagging is determined by the position of the nipple relative to the breast crease.
  • If volume is sufficient, the lift is done with your own tissue; if not, together with an implant.
  • Breastfeeding potential can be preserved with techniques that protect the milk ducts.

Understanding Sagging

Breast sagging is not a flaw but a natural consequence of life. Breast tissue that grows during pregnancy shrinks after breastfeeding ends, but the stretched skin cannot recover at the same pace. Weight fluctuations, genetics and smoking also reduce skin elasticity. As a result, the breast tissue descends and the nipple approaches or falls below the breast crease.

Why Isn’t an Implant Alone Enough?

An implant adds volume to the breast but does not lift sagging skin or a lowered nipple. When only an implant is placed in a breast with significant sagging, the implant may sit high while the tissues stay low. That is why a lift is essential if there is sagging; if volume is also desired, the two procedures are planned together.

Fullness with Your Own Tissue

In people with enough breast tissue, tissue from the lower part of the breast can be folded upward to create fullness in the upper area. This approach offers a natural alternative for women who do not want implants.

Timing

  • You should wait at least 6 months after breastfeeding ends.
  • Your weight should have been stable for at least a few months.
  • If you are planning a pregnancy in the near future, postponing the surgery is recommended.
  • After weight-loss surgery, you should wait for your weight to stabilize.

Step by Step

1

Planning

The incision type (around the nipple, vertical or inverted T) is determined by the degree of sagging.

2

Surgery

The breast tissue is shaped and lifted, excess skin is removed and the nipple is moved to its new position.

3

First weeks

A support bra is worn day and night, and heavy lifting is avoided.

4

Months 3 – 6

The shape settles and the scars begin to fade.

A Surgeon’s View

One of the most important factors that determine scar quality after a lift is care in the first months. Silicone gel or tapes and sun protection help the scars heal thinner and lighter.
